THE INDIVIDUAL EDUCATION PROGRAM (IEP)

The IEP Team includes:

An Administrator
The Student's teacher(s) *See note!
One or both parents
The student when appropriate
Other individuals
A Member of evaluation team


*(1) at least one regular education teacher if child is or might be participating in regular education environment
*(2) at least one special education teacher


THE IEP MUST INCLUDE:

Present Level of Performance

(1) how disability affects involvement and progress in General Curriculum

(2) for preschoolers, how disability affects participation in appropriate activities

Special Education & Related Services and supplementary aids & services to be provided and program modifications & supports provided for personnel to

(1) advance appropriately toward annual goals

(2) be involved in general curriculum & to participate in extracurricular and nonacademic activities.

Explanation of extent to which child WILL NOT participate in regular classes.

Modification needed to enable student to participate in regular state & district assessments, and if child will NOT participate, why the assessments are NOT appropriate, and how the child will be assessed.

Frequency & location of services & modifications

Beginning at age 14 (was 16), transition services focusing on child's course of study

Beginning 1 year before majority age, information regarding rights transfer to self

How progress toward annual goals will be measured and how parents will be informed of progress

Documentation of attendance of IEP team participants specifying their roles.
